What is the per-file limit?

Google and Bing both cap at 50,000 URLs and 50 MB uncompressed. Use a sitemap index when you exceed it. This tool warns above the limit.

Do priority and changefreq affect SEO?

Google has stated it largely ignores priority and changefreq. lastmod is respected. Bing and Yandex use changefreq as a hint.

Should hreflang go in the sitemap?

Yes. While in HTML works, sitemaps scale better for large sites and Google recommends it.
